An inquest jury decided on 15 September 2011 medical staff at Broadmoor had failed to carry out proper assessments or observations on double- killer Peter Bryan, allowing him to murder fellow patient Richard Loudwell in April 2004. Stephen Cragg acted for Mr Loudwell's family, instructed by Kate Maynard of Hickman and Rose".
