Jury finds systemic failings and unlawful killing in the inquest of 14-year-old Ruth Szymankiewicz detained under Mental Health Act
The jury in the inquest into the death of Ruth Szymankiewicz, a 14 year old girl detained under the Mental Health Act on a psychiatric intensive care unit, found systemic failings in her care on the part of Active Care Group contributed to her death. They also found that the failure to maintain her 1:1 constant observation levels amounted to unlawful killing.
